National Register HSP Credentialed Members: Submit this form to request that the National Register submit a verification letter to a licensing board, etc. Please enter the email address you have on file with the National Register when submitting this request. You must include your name and email address for us to process your request.

Employers and Healthcare Organizations: We’re now using an online verification system. Please create an account (with your organization email address) as a “credentials verification specialist” to verify a provider’s credentials. There is no charge to create an account for verifying provider credentials. Once your account is created, we will follow up with next steps.
