By checking the box agreeing to the National Register’s Terms of Service, I also hereby attest that all the information submitted on this application, and any further information required or requested in support of this application is true, correct and not misleading to the best of my knowledge. Permission is granted to contact any or all supervisors, educational and training institutions, licensing boards, and other relevant organizations (including their respective officers, directors, employees, and agents) that may possess any information and documentation both favorable and unfavorable needed to verify and/or clarify information concerning this application. I agree that there is no pending proceeding which may jeopardize my unrestricted psychology (or other mental health professional) license/certificate/registration in any jurisdiction. I have read the National Register’s membership and HSP credential benefits and understand that certain benefits such as licensure mobility, credentials banking and verification, and other specified benefits require the HSP credential, which requires a separate application process. I have read the HSP credentialing criteria and guidelines and understand that if I apply, my education, training & license(s) must meet these requirements before my application can be approved for issuance of the National Register Health Service Psychologist credential. I agree to be bound by and comply with the National Register’s Guidelines Concerning Withdrawal of National Register Membership and the HSP Credential Due to Professional Conduct
 

To maintain your National Register membership, you must comply with the following requirements:  

  1. Maintain active, doctoral level, unrestricted licensure/certification/registration in at least one US state/territory or Canadian province at the unlimited/independent practice level. 
  1. Submit the annual National Register membership renewal payment (non-refundable). 
  1. Complete and sign the annual attestation form that must be submitted with the membership renewal payment. In the interim, members must report in writing within 30 days any failure to meet criteria for membership, HSP credentialing or other violations of the Guidelines Concerning Withdrawal of National Register Membership and the HSP Credential Due to Professional Conduct
  1. Submit changes in address, phone, and e-mail within 30 days of the effective date, in order to ensure that all communications from the National Register are received.  
     

To obtain the HSP credential, you must comply with the following requirements: 

  1. Activate your National Register Membership (membership fees are non-refundable). 
  1. Submit the HSP credentialing application. For international degrees, a one-time $100 credentialing fee (non-refundable) will be incurred. 
  1. Arrange to have the required primary source documentation sent to the National Register. Please see your HSP credentialing application instructions for details. 
  1. An applicant’s education, training, and license must meet current HSP credentialing criteria and guidelines
  1. Upon review and approval of your HSP application and primary source verification submitted for credentialing, the National Register HSP credential will be awarded.  

To maintain your National Register HSP credential, you must comply with the following requirements:  

  1. Keep your National Register HSP Membership active and current. 
  1. Maintain active, doctoral level, unrestricted licensure/certification/registration in at least one US state/territory or Canadian province at the unlimited/independent practice level. 
  1. Submit the annual National Register membership renewal payment (non-refundable).

  1. Policy Statement 

Membership dues and fees paid to the National Register of Health Service Psychologists (National Register) are non-refundable except as expressly provided in this policy.  Payment of membership dues constitutes acceptance of the National Register’s membership terms of service and this Non-Refundable Membership Fee Policy. 

  1. Purpose  
    This policy establishes a consistent and equitable process for the collection and administration of membership fees while ensuring the National Register maintains the financial resources necessary to support member services, programs, education, advocacy, publications, technology, and operational expenses. 
  1. Scope 
    This policy applies to all membership categories offered by the National Register, including but not limited to: 
  • National Register Membership 
  • HSP Credential Membership 
  • Associate Membership 
  • International Affiliate Membership 
  • Retired or Emeritus Membership 
  • Organizational or Institutional Membership 
  • Any other membership category established by the National Register of Health Service Psychologists 
  1. Policy Provisions 
    4.1 Non-Refundable Fees 
  • All membership dues, late fees, application fees, credentialing fees, processing fees, reinstatement fees, and other membership related charges are non-refundable. 
  • Membership benefits become available upon processing of payment and initiation of membership, regardless of the extent to which the member utilizes those benefits. 
  • HSP credential benefits become available when the HSP credential is awarded.  If the HSP credentialing application is denied, not approved, requires additional information, or becomes inactive for any reason, HSP benefits are not awarded, and no membership fees will be refunded partially or in full. 

4.2 No Prorated Refunds 
The National Register does not provide full or partial refunds for: 

  • Voluntary resignation or cancellation 
  • Failure to use membership benefits 
  • Failure to maintain active, doctoral level, unrestricted licensure/certification/registration in at least one US state/territory or Canadian province at the unlimited/independent practice level 
  • Failure to earn HSP credential 
  • Change in employment, licensure, or professional status 
  • Relocation 
  • Retirement 
  • Dissatisfaction with National Register programs or services 
  • Any other reason deemed appropriate by the National Register 

4.3 Automatic Renewals 

  • Members enrolled in automatic renewal are responsible for managing or cancelling their recurring renewal authorization prior to the renewal date. 
  • Membership fees processed through automatic renewal are non-refundable once payment has been successfully processed. 

4.4 Membership Transfers 

  • Memberships are personal to the individual or organization under which they are issued and may not be transferred to another party or entity. 
  1. Membership Changes 
    Changes to membership category after payment generally apply to the next membership cycle unless otherwise determined by the National Register. 
  1. Exceptions 
    Refunds may be considered only under the following limited circumstances: 
  • Duplicate payments made in error 
  • Incorrect payment amount resulting from a National Register administrative or processing error 
  • Unauthorized credit card transaction verified by the financial institution 
  • Other extraordinary circumstances approved by the CEO or their designee 

Approved refunds, when applicable, will generally be issued using the original form of payment 

  1. Event, Registrations, and Product Purchases 
    This policy applies only to membership dues, membership related fees, and credentialing fees.  Refunds for educational programs, conferences, publications, merchandise, or other National Register products and services are governed by their respective refund policies or as deemed appropriate by the National Register of Health Service Psychologists. 
  1. Compliance and Policy Authority 
  • The National Register reserves the right to modify membership fees, membership categories, membership requirements, credentialing fees, credentialing categories, credentialing requirements, and this policy at any time. 
  • Any revisions shall apply prospectively unless otherwise stated.
