Reinstatements

If your HSP credential was discontinued within the past seven (7) years and you are actively licensed and have not had any disciplinary action, you may be eligible for reinstatement. To request reinstatement of your National Register Health Service Psychologist credential, complete and submit the Reinstatement Form (PDF)*.

*You may be asked to submit additional information to support your reinstatement request.
