The Substance Abuse and Mental Health Services Administration (SAMHSA) has published a series of tools for practices beginning to merge primary and behavioral healthcare.

The tools offered include checklists for steps to integration, evaluation of the existing groundwork to support integration, planning help, and a comparison of practice realities with the guiding principles of integrated care. Some tools can also help practices determine how integrated they are or can easily become. Others focus more on the goals of integrated primary care and what practices can do to better achieve them. The tools come from a variety of sources and are not endorsed by SAMHSA.
