Public Notices of Disciplinary Sanctions Against Registrants

Deletions Due to Suspension, Revocation, or Restriction of Only License.

Review by the National Register's Committee on Professional Practice and Ethics is pending to determine whether further action is warranted.

 

Deletions

The following persons have been deleted from listing with National Register pursuant to the "Guidelines." Public notices of final actions by COPPE appear on this web site for the period during which the person may not apply for reinstatement of his/her listing or reapply for listing. In cases where the person has been permanently barred from listing, the public notice is published for five (5) years.

BAUER, Janice L., Ph.D.: II.A. and II.F.; Deleted and barred from reapplication for two (2) years (November 13, 2008-November 12, 2010);  II.A.: Voluntary Surrender of New Hampshire license in settlement of pending disciplinary charges; II.F.: Failure to notify the National Register of the surrender of NH license.

BERZINS, Lisa G., Ph.D.:  II.A.; Deleted and barred from reinstatement/reapplication until the terms and conditions of the Connecticut Consent Order which directs a four year rehabilitation program for treatment of substance abuse are successfully completed; IIA: Connecticut license placed on probation.

CELIO, David L., Ph.D.: II.C. and II.F.; Permanently barred from reapplication, effective November 13, 2008; II.C.: Federal felony plea of guilty to health care fraud, resulting in imprisonment for 12 months and 1 day, followed by 2 years of supervised release, and fine of $120,000; II.F.: Failure to notify the National Register of charges by U.S. Attorney and failure to notify of plea to serious crime.

CERNY, Jerome A., Ph.D.: Sections II.A., II.B., and II.F.; Permanently barred from reapplication, effective February 11, 2005; II.A.: Indiana license revoked; II.B.: Violated Indiana code by engaging in lewd or immoral conduct; II.F.: Failure to notify the National Register of revocation of IN license.

DeVOGE, J. Thomas, Ph.D.: II.A., II.B., and II.F.; Deleted and barred from reapplication for two (2) years (November 13, 2008 – November 12, 2010); II.A.: Consented to reprimand of Ohio license and license restrictions with terms and conditions for 2 years; II.B.: Relinquished reasonable oversight and management of supervisee’s psychological work; II.F.: Failure to notify the National Register of reprimand and two-year restriction of OH license.

DOBBINS, Joel B., Ph.D.: II.A., II.B., II.C., and II.F.; Permanently barred from reapplication, effective January 7, 2008; II.A.: Texas license revoked; II.B. and II.C.: Pled guilty/nolo contendere to an indictment on two felony counts for indecency with a child under 17 years old; II.F.: Failure to notify the National Register of revocation of TX license.

GILL, Jamianne Dee, Ph.D.:  II.A., II.B., and II.F.; Deleted and barred from reapplication for five (5) years (November 13, 2008 – November 12, 2013); II.A.: Utah license placed on probation for 3 years with significant terms and conditions; II.B.: Cohabitated, traveled with, and engaged in an inappropriate romantic and physical relationship with a former client. Engaged in another second inappropriate dual relationship with a current client; II.F.: Failure to notify the National Register of the stayed revocation and probation of UT license.

JOSLIN, Gale L., Ph.D.:  II.A. and II.B.; Deleted and barred from reapplication for three (3) years (January 7, 2008 – January 6, 2011); II.A.: Texas license revoked; II.B.: Sexual misconduct, exploitation, and dual relationship with a patient.

LaBRAY, Ann, Psy.D.: II.A., II.B., and II.F.; Deleted and barred from reapplication for two (2) years (November 13, 2008 – November 12, 2010); II.A.: Reprimanded by Oregon Board of Psychologist Examiners with terms and conditions; II.B.: Stipulated to a failure to maintain professional standards for chart notes and to provide adequate informed consent procedures; II.F.: Failure to notify the National Register of reprimand and restrictions placed on OR license.

LORD, Dawn D., Ph.D.: Sections II.A., II.E., and II.F.; Deleted and barred from reinstatement/ reapplication until the terms and conditions of the August 31, 2004 Ohio Consent Agreement are successfully completed; II.A.: Ohio license suspended; II.E.: Admitted by Dr. Lord to the Ohio State Board of Psychology that she is an impaired psychologist; II.F.: Failure to notify the National Register of the suspension of OH license.Subsequently, OH license restored with terms and conditions as outlined in the April 15, 2005 Consent Agreement, including psychiatric treatment and practice monitoring with reports to the Board.

LUKENS, Horace C., Jr., Ph.D.: II.B.; Deleted and barred from reinstatement/reapplication for one (1) year (November 13, 2008 – November 12, 2009); II.B.: Recommendations not based on sufficient professional information.  Failure to obtain proper informed consent for the Independent Medical Examination (IME).

MCKEON, Patricia E., Ph.D.:  II.A., II.B., and II.F.; Deleted and barred from reapplication for five (5) years (January 7, 2008 – January 6, 2013); II.A.: California license revoked (revocation stayed) and placed on probation for five (5) years with terms and conditions; II.B.: Failure to respond to California Board of Psychology queries; Failure to provide written evaluation report upon patient’s request; II.F.: Failure to notify the National Register of stayed revocation and probation of CA license.

ORLANDO, Meryl A., Ph.D.: II.A. and II.B.; Deleted and barred from reapplication for two (2) years (March 5, 2008-March 4, 2010); II.A.: Ohio license given 30 day active suspension and 2 year practice restriction with terms and conditions; II.B.: Entered into a role conflict that substantially risked the presence of bias and impaired objectivity secondary to her prior evaluation and opinions rendered about members of a family.  Altered a signed release form.

PORTMAN, Sandra, Ph.D.:  II.A. and II.F.;  Deleted and barred from reapplication for three (3) years (July 27, 2007 – July 26, 2010); II.A.: Surrender of California license; II.F.: Failure to notify National Register of the surrender of CA license.

SHOFF, Susan P., Ph.D.: Sections II.A., II.B., and II.F.; Permanently barred from reapplication, effective February 11, 2005; II.A.: Surrender of California license under stipulated decision; II.B.: Gross negligence, repeated negligent acts, violation of Rules of Professional Conduct, and sexual relations with a patient; II.F.: Failure to notify the National Register of the surrender of CA license.

TARAN, Richard B., Ph.D.: Sections II.A., II.B., and II.F.; Deleted and barred from reinstatement/reapplication for five (5) years (February 11, 2005 - February 10, 2010); II.A.: Agreed to the suspension of Tennessee license for one (1) year followed by probation for two (2) years with conditions and restrictions; II.B.: Consented to professional misconduct; II.F.: Failure to notify the National Register of the suspension of TN license.
