Deletions

The following persons have been deleted from listing with National Register pursuant to the "Guidelines." Public notices of final actions by COPPE appear on this web site for the period during which the person may not apply for reinstatement of his/her listing or reapply for listing. In cases where the person has been permanently barred from listing, the public notice is published for five (5) years.

BERRY, George William, Ph.D.: II.A. and II.B.; Deleted and barred from reapplication for four (4) years (September 25, 2009 – September 24, 2013); II.A.: Indiana license on indefinite probation; II.B.: Failed to act within generally accepted ethical principles and guidelines of the profession; failed to maintain an awareness of personal and professional limitations.

BERZINS, Lisa G., PhD: II.A.; Deleted and barred from reinstatement/reapplication until the terms and conditions of the Connecticut Consent Order which directs a four year rehabilitation program for treatment of substance abuse are successfully completed; IIA: Connecticut license placed on probation.

CELIO, David L., PhD: II.C. and II.F.; Permanently barred from reapplication, effective November 13, 2008; II.C.: Federal felony plea of guilty to health care fraud, resulting in imprisonment for 12 months and 1 day, followed by 2 years of supervised release, and fine of $120,000; II.F.: Failure to notify the National Register of charges by U.S. Attorney and failure to notify of plea to serious crime.

DOBBINS, Joel B., PhD: II.A., II.B., II.C., and II.F.; Permanently barred from reapplication, effective January 7, 2008; II.A.: Texas license revoked; II.B. and II.C.: Pled guilty/nolo contendere to an indictment on two felony counts for indecency with a child under 17 years old; II.F.: Failure to notify the National Register of revocation of TX license.

GILL, Jamianne Dee, PhD: II.A., II.B., and II.F.; Deleted and barred from reapplication for five (5) years (November 13, 2008 – November 12, 2013); II.A.: Utah license placed on probation for 3 years with significant terms and conditions; II.B.: Cohabitated, traveled with, and engaged in an inappropriate romantic and physical relationship with a former client. Engaged in another second inappropriate dual relationship with a current client; II.F.: Failure to notify the National Register of the stayed revocation and probation of UT license.

GORMAN, John A., Ph.D.: II.A., II.B., and II.F.; Deleted and barred from reapplication for five (5) years (November 12, 2010 – November 11, 2015); II.A.: Voluntary Relinquishment of North Carolina license; II.B.:  Maintained a sexual relationship with the mother of a patient during the course of providing therapy to the minor child; terminated the therapeutic relationship with a patient for reasons unrelated to the patient’s needs; II.F.: Failed to notify the National Register of the surrender of NC license.

HAAPANEN, Randy Matt, Ph.D.: II.A., II.B., and II.F.; Deleted and barred from reapplication for three (3) years (November 12, 2010 – November 11, 2013); II.A.: Stayed revocation of California license with a three year period of probation; II.B.: Committed gross negligence by exposing a patient to explicit pornographic material and failing to provide patient with undivided attention during a therapy session; engaged in unprofessional conduct and provided suboptimal therapeutic services; II.F.: Failed to notify the National Register of suspended revocation and subsequent probation of CA license.

JAMES, Suzanne Day, Ph.D.:  II.A. and II.B.; Deleted and barred from reapplication for five (5) years (October 9, 2009 – October 8, 2014); II.A.: Restricted Minnesota license with terms and conditions including not accepting new DID clients; mandated supervision, personal therapy, education, and training; II.B.: Failed to meet the minimum standard of care for psychologists treating persons who are recovering childhood memories; failed to maintain appropriate boundaries with more than one client.

JOSLIN, Gale L., PhD: II.A. and II.B.; Deleted and barred from reapplication for three (3) years (January 7, 2008 – January 6, 2011); II.A.: Texas license revoked; II.B.: Sexual misconduct, exploitation, and dual relationship with a patient.

MAHONEY, David, Ph.D.: II.A., II.B., and II.F.; Deleted and barred from reinstatement/reapplication for one (1) year (November 12, 2010 – November 11, 2011); II.A.: Montana license placed on probation with supervision and mandated educational requirement; II.B.: Failed to meet generally accepted standards for licensed psychologists; failed to take reasonable steps to avoid harm; failed to avoid multiple relationships; failed to take reasonable steps to resolve multiple relations; used professional relationship to further a commercial enterprise for personal gain; II.F.: Failed to notify the National Register of the probation of MT license.

MCKEON, Patricia E., PhD: II.A., II.B., and II.F.; Deleted and barred from reapplication for five (5) years (January 7, 2008 – January 6, 2013); II.A.: California license revoked (revocation stayed) and placed on probation for five (5) years with terms and conditions; II.B.: Failure to respond to California Board of Psychology queries; Failure to provide written evaluation report upon patient’s request; II.F.: Failure to notify the National Register of stayed revocation and probation of CA license.

SIMMONS, Ronnie L., Ph.D.:  II.A. and II.F.: Deleted and barred from reapplication for five (5) years (November 12, 2010 – November 11, 2015); II.A.: Voluntary Surrender of Georgia license; II.F.: Failed to notify the National Register of Voluntary Surrender of GA license.

STREPPA, Michael S., Psy.D.: II.A., II.B., and II.F.: Permanently barred from reapplication, effective November 12, 2010; II.A.: North Carolina license summarily suspended leading to license revocation based on consent order; II.B.: Engaged in sexual intimacy with patient, including sexual intercourse; intimate relationship was abusive, inherently unequal in nature, and dangerous to patient; II.F.: Failed to notify the National Register of Consent Order.
